The American Indian Center at Mesa Community College has made your success our number one priority. Our goal is to provide culturally responsive academic support services that ensures academic success while embracing and affirming tribal culture and identities. All new students to MCC must complete the Student Information Form to get started.
Complete the Student Information Form:
There are two different ways to complete the Student Information Form (SIF)
The first option is to fill out the form online by visiting My.maricopa.edu. We highly recommend filling out this form online by clicking the link below.
If you need help, you can learn how to use My.Maricopa.edu.
The second option is to print the Student Information Form which you can then complete and submit manually.
Alternatively, you may also download and print the SIF form from a PDF file. Once completed, you can submit the printed form to Admissions & Records.

You will need to refer to the second page of the SIF for the Degree & Certificate codes. These codes will be used in order to complete the section about your educational goals.

Important Notes:
If you are a student outside of Maricopa County - you must complete an Out of County Affadavit to determine residency and tuition costs.
If you are under the age of 18 or concurrently enrolled in high school, you must complete the Concurrent Student Information Form to be admitted as a student with special status.
Former MCC students are not required to submit a new Student Information Form. Please follow the instructions for CURRENT or RETURNING students instead.
