Instructor Notification Forms

Complete the Instructor Notification Forms

An Instructor Notification Form will be issued to the student once reasonable accommodations are determined.

It is the responsibility of the student to obtain the Instructor Notification Form from a DRS Program Advisor to take to your instructor for their required signature. It is also the responsibility of the student to identify him/herself to their instructor that he/she has a disability and to discuss necessary accommodations. The form will help the student and the instructor discuss methods that will make the course fully accessible for the student's learning style. Asking for test accommodations one day or less from the exam is not reasonable notification by the student to the instructor.

If you haven't already done so, please schedule an appointment with your DRS Program Advisor to complete your Instructor Notification Forms prior to the start of classes.

IMPORTANT: The Instructor Notification Form MUST be completed each semester for each of your classes.

Visit the Disability Resources & Services office at Southern & Dobson or Red Mountain for help with your Instructor Notification Forms.
