  • Please use all lowercase letters for your username when you log into the form. If you use all CAPS, the program may log you in only as a user-level account.
  • At this time, the prioritize goals feature does not work.
  • MAC users may receive an error that they can only access the form on campus, when they are using a computer on campus. Please call the Helpdesk (1-7217) and ask for Dave to resolve this issue involving a DNS setting.
  • The form is currently not accessible through VPN / remote access.
